Healthy minds and mental health is a revolutionary status that we all must face everyday. We all face many problems everyday including terrorism, child abuse, rape, and murder and so forth whether indirectly or directly. There is no escape, so who is really out in this world with a healthy mind? NO ONE! If you honestly think that you are an exception to the rule then you are on the path to destruction. Even professionals are plagued with problems and if they say they are not, then you had better see someone else immediately.

The fact is most psychological professionals go into the business trying to escape their own realities in life. Many professionals would rather help someone else while burying their problems under the surface. Mental health related issues interrupt even professionals in the criminal system every single day. There is no one in this world excluded from mental illness.

There are all types of people in the world, starting with business related people who think they do not have a problem. Do you know what they do every night when they go home? Do you know how they handle their stressors? If you do, no need to read on, since I am out to revenge those that believe they have all the answers to each problem we face everyday of our lives, whether it is directly or indirectly.

I am out to avenge those that believe they are special and everyone around them has a problem. I am a realist, an expert in many fields and a woman that has gone all lengths to correct many opinions, theories, guesses and so forth. I do not believe that a person has the right to disregard another simply because they believe someone is imagining things. Rather, I will look closely at a person with mental illnesses and regard them before I will regard the so-called normal.

Let me hit you with my best shot. Contributes to Pat Benetar, we can see that there are many individuals in life and all of us have our own state of mind. We have schizophrenias, psychotics, bipolar, depressive, psychopathic, sociopath, and so forth. The problem is we are not agreeing on one belief. We have people that believe the law is right in all areas, but others that claim the law is controlling, which in fact, this second dis right in a large way.

In reality psychopathic and sociopath as well as other mental ill individuals are deemed GENUISES. Now we have a bunch of geniuses in life and a lot of average thinkers, so what are we going to do? When you are looking at geniuses verses common thinkers there are going to be conflicts, and each one will have their own state of mind devoid the other. We are talking CONTROVERSY!

Controversy? Let’s run like hell until we get there. When we do not understand something it is called IGNORANT. When we understand but refuse to accept it, is called STUPID. Now, if you think I am insulting, well, let’s take a closer look at what I am talking about and then you go ahead and judge me. OK. Ignorant, we have a problem; someone else does not see life the way we see life…HUM! Ok…I see it now. You are different from the other person you are talking to, and I am not seeing why this person thinks this way? HUM! Is it possible this person lived more than I lived? Does this person have more experience?

Is it possible this person has a medical related problem that is instigating their psychological impairment? Is it possible that the mind is affected and the body is not? What about the nervous system, how does it contribute to the mental status of the person? HUM! I am going to let you answer these questions yourself, since I do not have time to waste time.

But, after years of living, it should be obvious. We are talking about some real-life problems here and everyone is ignoring them, or disregarding them, stating that everyone should be treated the same. WRONG! Are you going to treat someone with Schizophrenia the same as you would treat a common mental ill individual? NO! IF you do, you are the one with more problems than the one that suffers Schizophrenia. The fact is these people are suffering and if you continue to ignore them, you will suffer too.

While you are sitting around blaming the victim, you might as well sit around and blame yourself. There are many cognitive disorders that we will discuss in the next article, however understanding which is cognitive and what is not can help us to understand mental illnesses.

There are those that walk amongst us who have followed all the advice, diligently dieted and exercised with genuine effort for long periods of time, yet still fail to lose their excess weight.

Even more heartbreaking for those of us who genuinely struggle with obesity are the accusations of sloth and laziness from those thin people who eat junk all day long and show no physical sign of their own gluttony. “Why can’t you just be like everyone else?”, they demand. “It must be all your own fault.”

“After all,” they say in derisory voice, “Losing weight is easy. Just eat less and exercise more. If you don’t lose weight, you are just weak willed.”

Every person who has never had a real problem thinks they know all the easy answers. All those underlying unwarranted generalisations we face from others just about every day of our lives is a little recognised yet very real and severe form of discrimination, based on the same types of wrong thinking as prejudice against race, skin color, creed, gender, and other well recognised forms of hatred.

In fact, life experience has taught me that the less a person actually knows about obesity and weight loss, the more they think they know.

I spent time a few years ago under the care of some of the most distinguished and knowledgable internationally renowned Professors researching weight loss and obesity from the University of Sydney and the Royal Prince Albert Hospital’s Obesity Clinic in Sydney, Australia. I will always remember what Professor Ian Caterson told me one day:

“The more we learn about obesity, the more we realise how little we really understand and how much more there is to discover.”

Would any of those permanently thin critics care to explain a man with whom I shared many physiotherapy and hydrotherapy sessions during my time on the program at the Obesity Clinic? He was normal down the left side of his body, though obese down the right side of his body only.

Next time someone verbally berates you for carrying excess body weight and tries to tell you how easy it is to be lean & trim, try asking them what they would recommend to that man that some of the top obesity researchers in the world don’t yet know.

Then, while your critic is looking dumbfounded and fumbling for some feeble explanation, hit them with a few more questions, such as:

1: Why do large birth weight babies have a dramatically higher rate of obesity than normal birth weight babies?

2: And, given the above, just what control do you think I’ve got over my birth weight?

3: Do you know what your metabolic rate is? If not, why do you presume to think you know mine?

4: Do you think everyone has identical norepinephrine levels? Oh, you don’t even know what norepinephrine is, huh?

5: Why do some people gain weight during stressful times while other people loss weight, irrespective of their eating patterns?

6: How do you expect an overweight person to even be capable of greater physical activity than a normal weight person?

7: Name me five minerals (not vitamins) that are vital for hormonal function, and explain which of these hormones has a different effect on brain signals related to appetite and metabolism.

8: Tell me what you know about Insulin Resistance Syndrome, what causes it, what its symptoms are, and how to treat it.

9: What has the least nutrient value to the human body – (a) 100 calories of pure fat, (b) 100 calories of pure protein, or (c) 100 calories of pure carbohydrate? (By the way – Your critic will probably answer Fat. The correct answer is C. The human body actually requires fats and proteins for various metabolic and physiological functions. Pure carbohydrate has no known unique use in the human body as the body can convert either fats or proteins as required into blood glucose, as it does with carbohydrate.)

10: Some of the most successful business people, scientists, philosophers and artists in the course of human history have struggled with obesity. Why do you think such successful and influential people who have changed the course of the world as we know it today are “weak willed”?

When you see the jaw of your critic drop to the floor, as they turn pale and look like they’ve been hit by a runaway train, when their silence shows that their ignorance and prejudice has been thoroughly exposed, just calmly walk away.

At this point, both of you now know that, even though you may carrying excessive body weight, it is your critic who has the bigger problem.

Tips for helping the family cope with critically ill patients

A significant role for the nurse is orientating a patient’s family to the critical care unit. Here are some useful tips for dealing with family members of critically ill patients.

The environmentIf it the first time the relatives have visited it is a good idea to greet the family at the door and explain to them what to expect. Ensure the relatives can see the patient by seating them on the side the patient is lying and lowering the bed if necessary. Make the relatives feel welcome and not a hindrance by introducing yourself and spending time with them. Explain what the equipment is for and what alarms mean. Explain the infection control measures.

Please touchLet family members know that it’s okay to touch the patient. Many are afraid to touch a critically ill loved one for fear of interfering with monitoring equipment or invasive lines. Let them know if there are any special considerations when touching the patient.

How’s the weather?Many family members spend their visitation time looking at equipment in the room and asking the patient questions such as, ‘Are you in pain?’ Encourage them to focus on the patient, not necessarily their pain or surroundings. You may have to explain on a daily basis how they should behave depending on changes in condition, for example, if the patient is agitated they should be calm and reassuring and ifthe patient is apathetic they should be upbeat and positive.

Let family members know how to be visitors.The patient wants to hear about the outside world – not reminders that they’re ill and hospitalised. They may want to hear about other family members, the family pet and who won the latest football match. It is important to encourage and motivate the patient and remind them about what they have to get better for.

One at a time, pleaseAsk the family to appoint one spokesperson for the group. This is especially important when families are large. The spokesperson is the person who should call the unit for updates on the patient’s condition. The spokesperson can then spread the word to the rest of the family. It may also be helpful to identify a primary nursing contact for the family.

Should they stay or should they go?Allow family members to stay at the patient’s bedside when appropriate. For example, a patient may require constant monitoring to keep them from trying to climb out of bed. If a family member is available to stay with such a patient, the use of sedatives and restraints could be avoided.

On the other hand, some patients appear to be agitated and have adverse changes in their vital signs when certain family members are present. Remember: Your first role is to be a patient advocate and to do what’s best for the patient. Ask them whether they want to have visitors and whom they want to visit. If having a family member is best for the patient, then allow the visitor to stay. Many units have open visitation policies but this should also be matched with needs of the relatives who should not be made to feel guilty if they find visiting difficult.

Ensure supportEnsure that support services are available to family members if they need them. If they belong to a particular church, offer to call someone if needed. Most facilities provide spiritual care for families if they request it.

Dengue is the most important arthropod-borne viral disease of public health significance. Compared with nine reporting countries in the 1950s, today the geographic distribution includes more than 100 countries worldwide. Many of these had not reported dengue for 20 or more years and several have no known history of the disease. The World Health Organization estimates that more than 2.5 billion people are at risk of dengue infection. First recognised in the 1950s, it has become a leading cause of child mortality in several Asian and South American countries.

This paper reviews the changing epidemiology of the disease, focusing on host and societal factors and drawing on national and regional journals as well as international publications. It does not include vaccine and vector issues. We have selected areas where the literature raises challenges to prevailing views and those that are key for improved service delivery in poor countries.

Shifts in modal age, rural spread, and social and biological determinants of race- and sex-related susceptibility have major implications for health services. Behavioural risk factors, individual determinants of outcome and leading indicators of severe illness are poorly understood, compromising effectiveness of control programmes. Early detection and case management practices were noted as a critical factor for survival. Inadequacy of sound statistical methods compromised conclusions on case fatality or disease-specific mortality rates, especially since the data were often based on hospitalised patients who actively sought care in tertiary centres.

Well-targeted operational research, such as population-based epidemiological studies with clear operational objectives, is urgently needed to make progress in control and prevention.

DF is typically acknowledged to be a childhood disease and is an important cause of paediatric hospitalisation in southeast Asia. There is, however, evidence of increasing incidence of DHF among older age groups. Since the early 1980s, several studies in both Latin America and southeast Asia have reported a higher association of DHF with older ages. The earliest studies were by Guzmán (1981) in Cuba and Rigau-Pérez in Puerto Rico .Later on similar observations were noted in Nicaragua and Brazil. In some southeast Asian countries where dengue has been epidemic for several years, this age shift is clearly observed, indicating an epidemiological change in dengue infection in those locations.

No studies suggest gender bias in home care and male preferences in health care seeking, still prevalent in many Asian and other traditional societies. It is widely recognised that in many of the Asian communities, lower disease incidence in women may be a statistical artefact related to lower reporting and care-seeking for women from traditional practitioners who do not report to public surveillance systems.
